Physical Examination

 
Students registering for athletics must have a current physical examination on file in the nurse’s office before the student is authorized to participate in practice/tryouts.  Physicals should be completed and turned in/uploaded to the Athletic Office no later than 15 business days prior to the first day of practice/tryouts. WINTER  REGISTRATION - NOVEMBER 11th)

 ***Important Information Regarding Registration for Season 2 and 3***

Health History must be updated 90 days prior to sport participation.  ALL VOLLEYBALL PLAYERS AND WRESTLERS must register again if registration was completed prior to December 1st 2020.

Physicals are valid for 12 months from date of the last exam.
